Recognising the disproportionate risk CVD poses to public health, EMPHN engaged POLAR to collaborate on a sophisticated and comprehensive CVD dashboard. This report provides an ensemble view of CVD at a practice population level and patient centric information.
Based on a live risk model, your practice can quickly and easily identify patients with an elevated risk of CVD. You can see the data quality issues that might affect individual scores, and export a list of current patients to allow for more targeted activity.
In developing this report POLAR was guided by best practice evidence which shows that small, measurable lifestyle changes are effective in reducing overall CVD risk. Our report creates interactive visualisations which assist general practice teams to empower and encourage at risk patients to address known risk factors.
