There’s nothing like POLAR. In addition to helping you support your practices, POLAR provides your PHN with advanced tools for visualising and analysing patient data from across your network. Best of all, any functionality or reporting generated from working with you on initiatives such as PIP-QI are provided to all PHNs at no extra cost. As our partner, your PHN will also be invited to provide regular feedback on updates and custom features to ensure POLAR continues to be aligned to the needs of your region.
Report with confidence
Intuitively visualise and monitor key priorities within a user-friendly, web-based interface made accessible across your entire network. Data is easily filtered to identify population health and service access trends across PHN catchments, providing insight for health planning activities, project development and evaluation.
Access clean, uniformed and secure data sets
POLAR cleans and unifies patient data, mapping GP data uniform to standardised terminology, such as SNOMED, to a single language for diagnosis, chronic disease, medications, pathology and radiology. With our data linkage tool it is then possible to link local data to other healthcare and hospital databases nationwide. All patient data is de-identified in our national database and securely hosted on local, fault-tolerant servers, which undergo regular enterprise-level security audits.
